    Once you have completed the application, print two copies of the page.

Qualified Wholesale Customers:

    Resellers, Entrepreneurs (with Vender I.D.), including educators as resellers.

Opening Account:

    $100.00 NET or $170.00 Retail (On first order only)

Reorders:

    Suggested $30.00 NET or $50.00 Retail

Freight:

    Orders shipped via UPS or Parcel Post.  An additional $1.50 will be charged for shipping and handling.

Backorders:

    Backorders under $25.00 NET will be cancelled unless otherwise requested.

Wholesale Discount:

    Wholesale customers receive a discount of 40% on all items ordered, unless otherwise stated.
